Section 29A of the Factory and Machinery Act 1967 ; stipulate that no person shall manufacture, fabricate, test, install, maintain, dismantle or repair any machinery which is prescribed unless a written authority has been issued by the chief inspector.                                        

Regulation 5(1) of the Petroleum Safety Measures (Transportation of Petroleum by Pipeline), Regulation 1985; stipulate that no person shall install or caused to be installed any pipeline unless he or his authorized representative has obtained a written permission from the approving authority.

Introduction To Unfired Pressure Vessels

Under the Factories and Machinery Act 1967, each unfired pressure vessel must have a valid certificate of fitness (CF) before it can be operated. For this purpose, approval for the design of the unfired pressure vessel has to be obtained in advance from the Department of Occupational Safety and Health (DOSH) before it fabricated, installed or used.

Application for approval of the design may be made by the manufacturing company, installer, supplier or owner of the unfired pressure vessel. Local manufacturers of unfired pressure vessels must register as a manufacturer with the department.

The following are several examples of unfired pressure vessels that require design approval:

1. Air container
2. Sterilizer
3. LPG storage tank
4. Heat Exchanger
5. Pressure vessels at the petrochemical plants etc

APPLICATION FOR DESIGN (STEAM BOILER, HOISTING MACHINES, UPV & LIFTS) AND PIPELINE APPROVAL - SKUD V3

Document Control and Manage System (SKUD) v3 is one of the online services provided by Department of Occupational Safety and Health (DOSH) to facilitate department’s client especially for steam boiler, hoisting machines, unfired pressure vessel (UPV), lifts and pipeline approval design application. This system is developed to be used by Design Section, Industrial Safety Division.

This system is a web-based system whereby it can be accessed through DOSH website. Department’s client can submit their application and documents needed through online at any places that have internet access. Every application submitted will be processed directly through the system by the officer in charge. The client can also check their application status and download the design approval letter through this system anytime and anywhere. This system also provides online payment facility to their client whether to use credit card or FPX facility.

However, the client must ensure the used browser because this system only can be viewed or accessed through Internet Explorer 7 and 8.

ONLINE APPLICATION SYSTEM [KLIK]

Sistem Aplikasi Atas Talian (OAS) merupakan portal yang menyediakan kemudahan kepada syarikat dan individu untuk memohon lesen dan perakuan melalui borang atas talian bagi jenis permohonan berikut:

1. Lesen Awam
2. Lesen Persendirian (Bagi Kapasiti Pepasangan 5MW ke atas)
3. Lesen Provisional
4. Lesen Penggunaan Gas
5. Lesen Gas Persendirian
6. Perakuan Kompetenan dan Pendaftaran Orang Kompeten Gas
7. Pendaftaran Kontraktor Gas
8. Kelulusan Peralatan Gas
9. Kelulusan Pengilang dan Pengimport Peralatan Gas
10. Kelulusan Untuk Memasang Sistem Talian Paip Gas
11. Kelulusan Untuk Mengendali Sistem Talian Paip Gas

Pihak syarikat dan individu perlu mendaftar sebagai pengguna sistem sebelum mengisi borang permohonan dan menghantarnya secara atas talian. Permohonan secara atas talian hanya sempurna apabila borang yang ditandatangan (hardcopy) dan lampiran lengkap (softcopy/hardcopy) diterima oleh pihak Suruhanjaya Tenaga untuk diproses.

Melalui OAS, pihak syarikat dan individu dapat menyemak perkembangan permohonan sama ada lulus atau sebaliknya. Disamping itu juga, OAS menyediakan kemudahan komunikasi atas talian bagi membantu pemohon untuk membuat sebarang pertanyaan dan keraguan pada permohonan yang memerlukan justifikasi terperinci.

OAS memudahkan pembekal data daripada pemegang Lesen Penggunaan Gas, pemegang Lesen Awam dan Lesen Persendirian Lebih 5MW menghantar laporan bulanan dan suku tahunan secara atas talian kepada Suruhanjaya Tenaga. Dengan OAS pembekal data akan menerima mesej peringatan untuk penghantaran laporan. Disamping itu juga, pihak pembekal data dapat menyemak laporan berkaitan untuk semakan dan pengesahan.
